Supportive Services Plan

Affordable housing has become a critical need for the growing deaf community in the San Francisco Bay Area, especially since the esteemed California School for the Deaf moved to Fremont in 1980. As they age, most deaf seniors become isolated from the larger deaf community due to a lack of transportation, money, and support systems. As the property management agent, Satellite Senior Homes will provide supportive services based on its experience of service provision at its other 16 senior sites. These services allow residents to age in place in their apartments. In addition, the Bay Area Coalition of Deaf Senior Citizens (BACDSC) and outside deaf service providers will offer services specifically targeted to the deaf. As evidenced by the supportive services plan below, Satellite and BACDSC are committed to the concept of integrating vital supportive services for seniors with the housing proposed in this application. Thus, the co-sponsors can assist residents to live comfortably in their own apartments and maintain their independence as long as possible.

As part of the supportive service plan, a service coordinator fluent in American Sign Language is available on-site to serve residents. The service coordinator provides case management and referral services to residents as well as coordinating specialized services offered by deaf service agencies on site on a periodic basis. Moreover, Satellite intends to duplicate service programs presently at its other facilities for the residents of Fremont Oak Gardens. These programs include van transportation, meal provision, nursing student health screening, intergenerational programming, and a tenant association with representatives on the Resident Council Of Satellite Senior Homes.

The Bay Area Coalition of Deaf Senior Citizens is committed to creating strong communication and networks and promoting coalition-building among deaf seniors to reverse the isolation of a large segment of the deaf senior population. BACDSC reaches 600 people per year with its newsletter and 500 people annually through its planned activities. The activities that will be made available on-site to residents at Fremont Oak Gardens include regional deaf senior clubs, fundraising and social events, newsletters, and working with the Deaf Seniors of America .

Both BACDSC and Satellite have far reaching ties throughout the Bay Area and in the Fremont community that enriches the delivery of supportive services to residents of Fremont Oak Gardens. Various service and community providers have committed their services, both on-site and off-site, to the residents of Fremont Oak Gardens. These providers include St. Joseph's Center for the Deaf, the Deaf Counseling, Advocacy, and Referral Agency (DCARA), the California School for the Deaf, Ohlone College Interpreter Preparation Program, Gallaudet University Regional Center, Washington Hospital Healthcare System, Legal Assistance for Seniors, the Deaf and Disabled Telecommunications Program (DDTP), Alameda County Social Services Agency, and Tax Signs. In addition, the City of Fremont Human Services Department and St. Anne's Episcopal Church located adjacent to the site intend to share programs in the most accommodating and accessible ways possible.
